Enter the date on which the tax information takes effect.

Select the type of tax card held by the employee.

Enter the current tax rate.

Select this check box to indicate that the employee is subject to withholding tax.

Select this check box to indicate that the employee is subject to non-withholding tax.

Enter the salary amount withheld monthly.

The employee identification.

Enter the non-taxed salary amount.

Enter the salary amount withheld monthly.

Enter the salary amount withheld weekly.

Enter the salary amount withheld daily.

Select the county in which the employee pays taxes.
