Use this form to set up specific work eligibility verification documents that employees provide to prove their work eligibility status.

There are three types of work eligibility verification documents:

  • List A– Official documents that establish both the person's identity and his or her eligibility to work. Some examples are a U.S. passport (unexpired or expired), an Alien Registration Receipt Card with photograph, or an unexpired Temporary Resident Card.

  • List B– Official documents that establish the person's identity only. Some examples are a U.S. driver's license, a voter's registration card, or an ID card issued by a local, state, or federal government agency.

  • List C– Official documents that establish employment eligibility. Some examples are a U.S. Social Security Card issued by the Social Security Administration or a Certification of Birth Abroad issued by the Department of State.

Depending on the employee's citizenship, Form I-9 information includes one List A document, or a combination of one List B document and one List C document.

Note Note

The check box controls whether the document type is available in the form. Occasionally, documents are removed from the list of acceptable documents that establish identity and work eligibility. When this happens, it is a good idea to inactivate the document type by clearing the check box.

Enter a unique identifier for the document.

Enter the official document title. For example, U.S. passport.

If applicable, enter the official identification number assigned to the Immigration and Naturalization Service (INS) Form.

For example:

  • INS Form I-151 of I-551 (for a Temporary Resident Card)

  • INS Form I-766 (for an Employment Authorization Document)

Select the government authority that is authorized to issue the document.

Indicate whether this is a List A, List B, or List C document.

Enter the expiration date of the document.

Control whether the document type can be selected as a Form I-9 verification document in the form.
