There isn't a Main Menu form (or memory to load it) in this dictionary.
Dexterity couldn't locate the Main Menu form or ran out of memory trying to load the form.
Be sure the dictionary has a Main Menu form and that the form name is spelled correctly. The name must be "Main Menu". If you think Dexterity is running out of memory, remove some fields from the main window.
