Use this form to reverify employment eligibility for an employee within three years of the date on which the employee supplied the information. After you enter and sign the reverification information, a new record is automatically created in the form.

The name of the person who prepared the information on behalf of the employee.

Verified

The date on which the Form I-9 was verified by the employer. This date is automatically assigned when you use the button to confirm the accuracy of the information.

The date until which the person is eligible to work.

Enter the official alien or admission number, depending on the verification document that the person provides.

Select this option if the person is providing a List A document. You can then enter information about the document.

Select the type of verification document that the employee is providing.

Enter the official document number of the verification document.

Select the date on which the document will expire.

Select this option if the person is providing a List C document. You can then enter information about the document.
