Use this form to enter new Form I-9 work eligibility information about an employee, or to reverify what the person has already provided.

If the employee required assistance to complete the form, such as translation assistance service, you can enter contact information about the preparer on the tab.

You can then print the information in a report that is formatted to resemble the official form issued by the government by clicking Print.

Note Note

After the information is marked as verified, you cannot make changes and the document cannot be printed again.


If you discover an error, you can reset certain information in the form by clicking > . When you reset the form, information in the , , , and fields is reset. Furthermore, you can modify the document type, the document number, and the expire date.

The date on which the Form I-9 information was prepared for the employee. This is relevant, for example, if the employee required the assistance of a translator in order to complete the form. This date is automatically added to the document when the preparer signs the document.

Select the employee who verified the Form I-9 information.

The date on which the Form I-9 was verified.

The name of the employee.

The job title of the employee.

The date until which the employee is eligible to work.

The official alien or admission number of the employee.

Select this option to enter information about a List A document.

Select the type of verification document provided by the employee.

Enter the official document number for the Form I-9.

Select the date on which the document will expire.

Select this option to enter information about a List B and a List C document.

Select the type of verification document provided by the employee.

Enter the name of the person who helped the employee complete the Form I-9.

Enter the city in which the person who helped the employee complete the Form I-9 lives.
